Public Health and Health Professions 2020 goals.
The priorities for DPHHP in developing the Public Health Strategic Framework are:
- to address the socio-economic and cultural determinants of ill health and reduce inequalities in health
- to strengthen public health services to support the improvement of health in local communities and commission health and social care
- to further improve the health of children and young people to give them the best start in life
- to tackle obesity by encouraging healthy eating and fitness
- to address health-related behaviours and risk (including smoking, alcohol, drugs, sexual health)
- to better manage long-term health conditions and reduce their impacts on health and well being
- to improve the delivery of mental health services.
To this end, the Penarth Integrated Care team was developed to facilitate prompt medical services and the reduction in health service inequalities to a given patient demographic/population. The team was founded in early 2008 to assist patients within nursing homes attain regular sustainable General Practice healthcare contact. At this time (early 2008), the team was known as 'The Winter Pressures Team' and was focused on alleviating pressure on primary care services by forward planning and supporting older people in nursing homes.
However, one year in, the team has undergone rapid development and is now known as the Penarth Integrated Care Team or PICT, the team now utilises both medical and nursing professions to manage chronic health conditions as well as short term interventions on a presentational basis. As a team we work for all four surgeries in Penarth but have an office in Stanwell Road surgery.
From a nursing perspective Mrs Joanne Humphreys RGN works for the team on a Monday, Tuesday and Wednesday whereas Mr Rich Davies RN covers Wednesday, Thursday and Fridays.
Dr Kath Bilton is the team general practitioner and works Mondays, Wednesdays and Fridays, on these days she is responsibe for chronic and acute management of an increasing number of patients living within nursing homes in the CF64 catchment area.
