Referrals

We accept referrals from the GP surgeries/Ambulance trust/Social services and secondary care that meet any the following criteria:

1. Housebound patients with complex needs that require a Level 1 and 2 assessment.
2. Residential Home patients with complex needs that require a Level 1 and 2 assessment.
3. Housebound or Residential Home patients with suspected UTI, chest infections or minor illness. (Richard only)
4. Housebound or Residential Home patients with constipation or minor illness. (Jo only)
5. Nursing home patients with medical needs can be reviewed by Dr Bilton on Monday, Wednesday and Fridays.

Referrals can be made either via the relevant surgery or by telephoning to the nurse on duty.

Connections

The team has secured relationships with a variety of multidisciplinary services and continue to make connections with other agencies. This enables referrals to these agencies to be given a priority status.
