Who are we?
PICT are a Local Health Board (LHB) funded initiative that works in conjunction with the 4 GP practices in Penarth to improve the health care of older people. We comprise of 1 full time nurse, 1 part time nurse and 1 full time doctor.

Aims
- To improve the health and well being of elderly patients
- To co-ordinate and deliver multidisciplinary services in a primary care setting
- To reduce unplanned contacts with the out of hours provider and ambulance services
- To reduce unplanned hospital admissions
Who we see?
- Nursing home patients
- Residential Home patients
- Elderly patients with needs in the community
- Patients with acute needs (Referrals via GP only)
- Housebound patients of varying ages
